Bruxism ranks among the most overlooked conditions in dentistry — mostly because people do it while asleep and never realize it. By the time I notice the telltale wear on a patient's teeth, the harm has often been piling up for years. The encouraging part: once we catch it, it's straightforward to guard against. Here's what you need to know.
What Is Bruxism?
Bruxism means clenching, grinding, or gnashing the teeth involuntarily — most often while asleep (sleep bruxism), though it also happens during the day under stress (awake bruxism). Roughly 8–10% of adults deal with it, and it shows up more in people who:
- Live with a lot of stress or anxiety
- Have sleep apnea or another sleep disorder
- Drink a lot of caffeine, especially later in the day
- Use tobacco or alcohol
- Take certain antidepressants (SSRIs)
- Have a bite that's out of alignment (the teeth don't meet evenly)
Signs You May Be Grinding — What Dr. C Looks For
- Jaw soreness or tightness in the morning
- Headaches centered around the temples
- Earache with no ear infection
- Worn-out facial muscles
- More tooth sensitivity than before
- A jaw joint that clicks or pops
- Disrupted sleep — yours or your partner's
- Flattened, worn-down biting surfaces
- Teeth looking shorter than they once did
- Chipped or cracked enamel
- Scalloped indentations along the tongue's edge
- Ridges in the cheek tissue inside
- Enamel worn through to yellowish dentin
- Cracked or broken restorations
The Damage Bruxism Does Over Time
This is the part that worries me most when bruxism goes unchecked. Grinding can generate force 10–40 times what normal chewing produces. Across years, that means:
- Enamel grinds flat — and enamel never regrows. Once it's worn away, the dentin underneath is left exposed, causing sensitivity and even faster wear
- Teeth chip and crack — stress fractures form in the enamel, leading to chips or full cracks that may need crowns
- Restorations give out early — fillings, crowns, veneers, and implant crowns are all at the mercy of grinding's excess force
- TMJ damage — constantly overworking the jaw joint can lead to arthritis, a displaced disc, and lasting pain that's hard to undo
- Bone loss around implants — grinding is among the top reasons implants fail in otherwise healthy patients
Treatment — Custom Night Guard vs Store-Bought
A custom dental night guard built from a digital scan of your bite is the strongest protection going. Here's why custom outdoes over-the-counter:
| Feature | Custom (Merry Dental Hub) | Store-Bought |
|---|---|---|
| Fit precision | Exact fit from a digital scan | Generic — never quite right |
| Comfort | Most patients forget it's there | Bulky and often uncomfortable |
| Jaw alignment | Tailored to your specific bite | Can make TMJ symptoms worse |
| Durability | 3–5+ years | Often only weeks to months |
| Cost | $400–$700 (often insured) | $20–$80 |
