Whenever I lay out a treatment plan, "Do I actually need a deep cleaning?" tends to be the first thing patients ask — and rightly so. The names are confusing, and you deserve to know what you're paying for and why. So let me give you the straight version.
Regular Cleaning (Prophylaxis) — For Healthy Gums
The everyday cleaning — its clinical name is prophylaxis — clears plaque and tartar off the tooth surfaces above and just at the gumline. Think of it as a preventive visit for people whose gum tissue is healthy and whose pocket depths sit in the normal 1–3mm range.
- Scaling — clearing plaque and tartar from every tooth surface sitting above the gumline
- Polishing — lifting surface stains with prophylaxis paste
- Flossing — cleaning the contacts between teeth
- Fluoride application (optional) — reinforcing the enamel

Deep Cleaning (Scaling & Root Planing) — For Gum Disease
A deep cleaning — known clinically as scaling and root planing (SRP) — is a therapy rather than routine upkeep. It comes into play once a patient develops periodontitis: gum disease in which pockets have deepened past 4mm and tartar has lodged below the gumline, beyond the reach of standard instruments.
Here's the crucial distinction: scaling and root planing works beneath the gumline, using purpose-built instruments to:
- Local anesthetic is given so the area is completely numb
- Scaling — clearing tartar and bacteria below the gumline, right down to the bottom of each pocket
- Root planing — smoothing the root surfaces so bacteria struggle to re-attach
- An antibiotic agent may be placed into the deepest pockets
Duration: 1–1.5 hrs per session · Sessions: Often 2 (split by side or quadrant) · Insurance: Typically 50–80% on PPO plans · Cost: $150–$350/quadrant
